After a tragic incident at Stratford London Underground station, where a 72-year-old man, Brian Mitchell, lost his life after being run over by multiple trains, a coroner has highlighted the need for increased vigilance among train operators using automated driving systems. The incident occurred on Boxing Day 2023 when Mitchell, reportedly intoxicated, inadvertently fell onto the tracks. Despite attempts to climb back onto the platform, he was struck by an oncoming train, leading to further tragic events.

The coroner’s report emphasized the importance of train operators closely monitoring the tracks and being ready to override automated systems if necessary. It was noted that with proper attention, Mitchell’s presence could have been detected, and the tragedy potentially averted.
